The report is the main document with the help of which the serviceman addresses the command . Through the report, the serviceman requests leave, referral for treatment, dismissal from service, transfer to another unit, referral to the VLK, payment of funds, etc.

As for the preparation of the report, there are no established requirements from the legislation that must be followed when preparing the report. There are only generally accepted rules (customs) of office management, according to which the following should be noted in the report :

  • the commander of the military unit (usually all reports are addressed to him);
  • Name of serviceman;
  • the number of a military ticket or passport, or RNOCPP;
  • content of the report itself (indication of the circumstances);
  • cool part;
  • date, initials, signature.

The report can be submitted in the following ways :

  • by command - submission of a report by place of service to the office, immediate superior or clerk of the military unit;
  • by post (Ukrposhta or courier delivery).

Always when submitting a report, it is desirable to receive confirmation of its submission . If the report is submitted by command, then two sets must be made, the first is submitted, and a mark of receipt is placed on the second (a stamp or seal, or the signature of the responsible person). As for postal communication, the best way is to send a valuable letter by Ukrposhta with a description of the attachment , which indicates the name and date of the report, a list of attachments. All this is necessary in order to at least not lose time, because usually a lot of reports are submitted and there are rare cases when they are trivially lost, so the serviceman is forced to submit the report again and wait for its resolution. In addition, it happens when unscrupulous commanders deliberately "do not allow the progress of the report" for various reasons and, due to the lack of confirmation of the submission of the report, avoid responsibility.

As for the appendices to the report, ordinary copies (military ID card, medical certificates, letters) are usually sufficient when submitting the report, the commander may ask to provide the originals to verify the authenticity of the document already during the review of the report.

The general term of response to the report is 15 days, which can be extended up to 30 days . In the report, it is possible to indicate how you would like to receive an answer (phone, e-mail, in writing), in any case, you have the right to receive a written answer , in case of disagreement with it, appeal it through higher authorities or a court.
